Board of Directors

The Board of Directors was formally established on May 7th, 2008 and is made up of 9 to 21 members, at least 51% of which are patients of The Lakes. The Board meets monthly.

    • Insurance Information
    • List of medications, vitamins, supplements you are currently taking
    • For sliding fee scale applicants: application form and proof of income
